The 64-66 template is for the optional right side mirror part # C4SZ-17696-A, both these kits originally included mounting screws but I have not seen any of the inserts that where used particularly on the 64-66. When I am doing a restoration and the car is striped down I install a reinforcing plate inside the door using body panel adhesive to fasten it in place.
